El cristalino es una lente natural que forma parte del ojo, se encuentra detrás del iris y sirve para enfocar las imágenes como lo hace el zoom de una cámara. Con el tiempo, el cristalino pierde su capacidad de enfoque y aparece la presbicia o vista cansada, que se manifiesta por la imposibilidad de ver de cerca. Además, el cristalino va perdiendo su trasparencia según envejece y se va volviendo opaco, provocando lo que llamamos cataratas.

Para estas patologías la solución es el tratamiento con lente intraocular (también llamadas lentes pseudofáquicas). La operación consiste en extraer el cristalino dañado y sustituirlo por una lente intraocular, que hace la función de cristalino artificial.

En la actualidad, estamos implantando las lentes intraoculares multifocales, que como su nombre indica, permiten además adaptar la visión en varios planos (entre lejos, intermedio y cerca), siendo el cerebro el que elige el enfoque visual más adecuado para cada situación gracias a un procedimiento oftalmológico denominado pseudoacomodación. De esta forma, los pacientes recuperan un amplio rango de visión y pueden realizar actividades sin necesidad de utilizar gafas o lentillas, desde leer un libro hasta conducir. Este tratamiento permite, además, eliminar defectos visuales asociados como la miopía, la hipermetropía y el astigmatismo.

The crystalline lens is part of the eye and sits behind the iris. Its purpose is to focus on images, just like the zoom of a camera. With time, the crystalline lens loses its ability to focus; this inability to see nearby objects is termed presbyopia, which is also known as long-sightedness. On the other hand, as people get older, their crystalline lenses gradually lose their clarity until they become milky white. This is what we call cataracts.

The solution for these pathologies is a treatment with intraocular lenses (also known as pseudophakic lenses). The operation consists in removing the damaged crystalline lens and replacing it with an artificial intraocular lens which will act as the former.

Clínica Baviera has extensive experience in intraocular lens implantation. Curently, we are implanting multifocal IOLs, as the name implies, which allows focusing our vision on different planes.

However, for the past eight years we have implanted multifocal intraocular lenses in thousands of operations. As their name implies, these lenses allow the eye to focus on several planes (far-off, intermediate, and nearby), the brain choosing the most suitable focus for each situation thanks to a process known as pseudoaccommodation. Thus, patients recover a wide range of sight and can do all kinds of activities without having to wear glasses or contact lenses, from reading books to driving. This treatment also allows doing away with associated defects such as myopia, hyperopia and astigmatism.

Cirugía con lente intraocular. El tratamiento paso a paso

El preoperatorio.
Una semana antes de la cita, el paciente deberá dejar de usar las lentes de contacto si son blandas y, en caso de usar lentillas duras, durante dos semanas. Además no podrá maquillarse durante una semana, tal y como también se recomienda en las operaciones de cirugía refractiva láser para corregir miopía, hipermetropía y/o astigmatismo a través de diferentes técnicas y dejar de usar gafas y lentes de contacto.
Una vez llegado a la clínica (preferiblemente por la mañana), al paciente se le realizarán una serie de pruebas preoperatorias que serán revisadas por un oftalmólogo que decidirá la idoneidad de la cirugía y la técnica a utilizar durante el tratamiento. Esta consulta preoperatoria durará unas dos horas.
La operación.
La operación habitualmente se realizará al día siguiente de la visita. Debe tener en cuenta que al tratarse de lentes intraoculares pseudofáquicas personalizadas (trifocales, multifocales o monofocales) y adaptadas a las necesidades de cada paciente, el tratamiento se puede demorar unos días más hasta que tengamos las lentes adecuadas para su caso por lo que el tiempo de estancia podría alargarse. Se instaurará el tratamiento preoperatorio, con unos colirios (gotas). En este tipo de intervenciones se puede plantear realizar la cirugía el mismo día en ambos ojos, o bien dejando un intervalo de 24 horas entre la operación de un ojo y el otro; será el cirujano especialista el que decida el tiempo que debe transcurrir entre las intervenciones de los dos ojos según cada caso en particular.

La intervención comienza con la aplicación de un colirio anestésico (gotas) y con la inmovilización de los párpados del paciente para evitar el parpadeo.
El primer paso de la intervención consiste en disolver y extraer el cristalino deteriorado a través de la utilización de ultrasonidos mediante una técnica llamada facoemulsificación. A continuación, el cristalino se sustituye por una lente intraocular (cristalino artificial), que dura toda la vida y no pierde transparencia, a diferencia de lo que ha ocurrido con el cristalino natural del ojo. De esta forma, mediante la facoemulsificación y el implante de un cristalino artificial multifocal eliminamos las cataratas (si las hubiese), corregimos la presbicia o vista cansada, y podemos eliminar o minimizar cualquier problema refractivo asociado (miopía, hipermetropía, astigmatismo). En el caso de que el paciente no padeciera cataratas, con esta técnica se evitará su aparición. Una vez realizado este procedimiento, el paciente puede irse a casa con el ojo destapado y su recuperación visual será muy rápida desde la operación.

Implantación lentes intraoculares

El postoperatorio.

Tras la intervención, el paciente permanece en reposo en la clínica durante, aproximadamente unos 20 minutos durante los cuales el oftalmólogo realiza una primera revisión ocular. Una vez transcurrido este tiempo, el paciente abandona la clínica por su propio pie.

El postoperatorio es totalmente indoloro y muy rápido y al día siguiente de la intervención el paciente que se ha realizado la cirugía puede comenzar a hacer actividades cotidianas, manteniendo unos cuidados básicos (que le serán explicados adecuadamente por el equipo oftalmológico).

La primera revisión postoperatoria será al día siguiente de la operación (estancia media de 4 días). Nuestros profesionales le darán las recomendaciones necesarias para tener una recuperación de la visión óptima y se realizará un control posterior a la cirugía vía mail/teléfono a los 7 días de la operación.

El oftalmólogo le prescribirá los fármacos que considere necesarios, para después de la operación.

Entre un mes y tres meses más tarde deberá volver a nuestra clínica para una última revisión en la que confirmaremos si todo ha salido bien, en cuyo caso le daríamos el alta definitiva tras la intervención.

Asimismo, tendrá a su disposición a un especialista para consultarle vía telefónica o mail cualquier duda que le pueda surgir.

Intraocular lens surgery the treatment step by step

The preoperative phase.
If the patient wears soft contact lenses, he will have to stop using them one week before the appointment; in the case of hard contact lenses, two weeks earlier. Patients are not allowed to wear makeup one week prior to the appointment.
Once the patient arrives at the clinic (preferably in the morning), he will undergo a series of preoperative tests whose results will be checked by an ophthalmologist, who will decide whether surgery is appropriate and, if so, the technique to be used. This preoperative appointment lasts two hours.
The operation.
The operation is usually done the day following the day of the visit. Bear in mind that given they are customised lenses, the treatment can take a few days longer in order to get the lenses which are right for you, so that the stay could take a bit more time. Eye drops are prescribed as preoperative care. There are two options for this type of operation: undergoing bilateral surgery, i.e. doing both eyes on the same day, or operating one eye first and letting 24h pass before moving to the other; depending on each individual case, the specialist surgeon will decide how much time must elapse between both operations.

The operation starts with the application of anaesthetic eye drops and the immobilisation of the eyelids to prevent the patient from blinking.

The first step of the operation is to dissolve the deteriorated crystalline lens with ultrasounds and remove it by means of a technique known as phacoemulsification. Next, the crystalline lens is replaced with an intraocular lens (artificial crystalline lens) which will last a lifetime and will not get opacify. Thus, thanks to the artificial multifocal crystalline lens cataracts (if any) are eliminated, presbyopia is corrected and any associated refractive issue (myopia, hyperopia or astigmatism) is solved or minimised. In case that the patient does not suffer cataracts, this technique will prevent them. After this procedure, the patient can go home with the uncovered eye and his recovery will be very fast.

Intraocular lens surgery

The postoperative phase.

After the operation, the patient will remain at rest at the clinic for about 20 minutes, during which the ophthalmologist will perform a first eye examination. Once this time is over, the patient will be discharged.

The postoperative phase is completely painless and very quick. The patient may begin to perform everyday activities the day after the operation provided he takes a few basic precautions (which the ophthalmology team will explain in detail).

The first postoperative check will take place the day following that of the operation (average stay: 4 days). Our doctors will issue a series of necessary recommendations to the patient for him to have an optimal recovery. Seven days after the operation they will perform a second postoperative check over the phone or by e-mail.

The ophthalmologist will prescribe the appropriate drugs for the postoperative phase.

Between one month and three months later, the patient must return to the clinic to undergo a last check-up to verify that everything went fine, in which case he would be permanently discharged.

Moreover, there will be an ophthalmologist at the patient's disposal to solve any questions the latter might have by phone or e-mail.
